The Fort Worth Hispanic Chamber of Commerce and UnitedHealthcare, Latino Health Solutions would like to cordially invite you to their upcoming August Business After Hours Mixer / Hispanic Health Leaders Award Reception. The event will be held on Thursday, August 18th, 2011 from 5:30 to 7:30 pm.

Just like last year, we will present eight Health Care Professional Awards to individuals / organizations that have performed outstanding service in their profession while making a difference on behalf of the Hispanic community. The categories are as follows: Community Health Care Worker, Dentist, Doctor, Dr. Adela N. Gonzalez Volunteer of the Year, Hospital, Clinic and/or Organization, Health Care Worker (Physical Therapist, Paramedic, Other), Nurse, and Pharmacist.

In order for us to accurately identify and celebrate the accomplishments of these individuals/organizations, we would really appreciate if you could select your own nominees and mail the completed form to 1327 N Main Street, Fort Worth, TX, 76164, attention to Health Care Awards Nominations, or e-mail completed forms to Kathryn Johnson (kathryn.johnson@fwhcc.org), subject line Health Care Awards Nominations, no later than 5:00 PM on Friday, August 5th, 2011.
